SERVICES, POLICIES & FEES

We provide a full family medicine experience for patients of all ages. Please consult our fee schedule below and ask the office staff for details. 

 

Medical Emergencies
Patients with serious medical emergencies should go to the nearest Emergency Department.

 

No-Show and Late Fees
We charge $75 for missed complete physical examination appointments with Dr. Carson or Dr. Sheng if 24 hours notice of cancellation is not given.

 

Scheduling and Office Appointments
Regular visits: 15 minutes
PC1 Appointments: 30 minutes 

If a patient has a problem that will require a long visit, the front desk must be informed at the time of booking.

Patients must bring and present a valid (green - NOT red and white) OHIP card to all visits. The government will not cover medical services for anyone without a valid health card. For non-OHIP patients, fees have to be paid prior to leaving the office after seeing the doctor. Official receipts are provided upon request.

 

Uninsured Services
We have an ANNUAL SERVICE FEE of $110 per family or $80 per individual (both plus HST). This offers patients the prepaid advantage of NOT being billed per service for items below that OHIP will not pay for. These include: prescription repeats when necessary; telephone advice; short forms, notes and letters (not legal or insurance). Record transfer fees will still be charged.

PRIVACY POLICY

Email Service 

We still have our Gmail messaging system, however, we will no longer send medical information by Gmail to you. When we request information from patients (i.e blood pressure charts or photos), or send you private health information (i.e test results) we use our new secured system, OceanMD. Please let the office staff know if there are any changes to your email address so we can update the system with your correct information. DO NOT send health questions by Gmail, as it is not a secure platform.  In most cases you should call our office to book an appointment.

 

Messaging our office:

  • Should not be used for urgent matters

  • Should not be used for prescription renewal requests.

  • Will be opened by the front desk staff.

  • Will be acknowledged by Dr Carson & Dr Sheng's office/Sport Med North within 2 business days.

 

 

Messages from Sport Med North:

  • Will not be used for urgent matters.

  • May be used to periodically communicate matters such as drug recalls, Newsletters and updates.

  • Will use the 'blind copy feature' to send bulk emails (i.e: Newsletters) to maintain privacy.

 

 

Important Information Regarding Messaging / Email Communication

  • Messages containing important health or legal information will be stored in the patient's file.

  • Do not rely on email communication for emergencies, urgent instructions, or your medical concerns. 

 

 

Email addresses or patient information will NOT be shared with third parties without the patient's expressed permission. All information provided in the emails will be kept confidential.
